SUMMARY:Bootcamp III: Advance Care Planning
DESCRIPTION:Please join us for the third in our quarterly series of Bootcamp Trainings\, designed to assist primary care practices in implementing best practices in geriatric care. This Bootcamp will cover topics associated with Advance Care Planning\, and the content will be of benefit to everyone within the practice team. There will be some skills-based training in the morning on holding effective and efficient conversations for individuals with serious illness. All providers who may be facilitating ACP in their practice are encouraged to attend this Bootcamp\, as well as all members of the implementation team. The Bootcamp will be held at the Audubon McLane Center at 84 Silk Farm Road in Concord\, New Hampshire.
